The BT-4 is a bag of five large clear plastic earcones for the RTS ET-4 coiled acoustic eartube. The earcone is the part that sits in the ear, so it is the piece that wears or gets lost first, and the ET-4 ships with one of each size. The BT-2 is the small and the BT-3 the medium earcone.
